The link was posted on the Pattaya City Expats Club home page late evening on 8/24/21 based an email they received from Tanya at BHP - https://docs.google.com/forms/d/e/1FAIp ... 3769667344

For some reason BHP keeps calling their registration pages a "survey" even though it is actually for signing up for the vaccination. After registration, they send a message advising of the time and date for your appointment. The vaccination location is Central Festival Pattaya Beach - not at the hospital. They have a space for your Hospital Number, but IT IS NOT A REQUIRED FIELD.

It also shows choices for vaccine:
Please select your preferred vaccination program. (You could select more than one program) *
Program 1: Pfizer (1st dose) + Pfizer (2nd dose)
Program 2: AstraZeneca (1st dose) + AstraZeneca (2nd dose)
Program 3: Sinovac (1st dose) + AstraZeneca (2nd dose)
Program 4: Sinovac (1st dose) + Pfizer (2nd dose)

I wonder why one of the choices is not AstraZeneca followed by Pfizer.

I answered the survey requesting 2 Pfizer doses this is how they replied:

You have confused the BHP's private sale of Moderna, which ended a few minutes ago, with the public provision of Sinovac, A-Z, and Pfizer which they handle as one of many contractors to the Ministry of Public Health.
