A US destroyer has collided with an oil tanker off Singapore. 10 sailors remain missing. The destroyer, the USS John S. McCain, is named after Senator John McCain's father. This is the fourth incident in the Pacific Ocean this year involving a collision between a US navy vessel and a merchant ship. The worst was just two months ago when the USS Fitzgerald collided with a container ship off Japan, an accident that killed seven sailors. Four days ago it was announced that 12 members of the crew will be disciplined, including the Commanding Officer and two other senior crew.

How is it that some of the most sophisticated warships on the ocean do not have systems on board to avoid recently crashing into much larger merchant vessels?
